DOI: https://doi.org/To explore how the National Multisectoral Health Policy (PNMS) affects the governance of regional health directors in Peru in the year 2025. A qualitative approach was used, with an inductive method and a multiple case study phenomenological design. Nine regional directors were interviewed through semi-structured guides, and the analysis was carried out using ATLAS.ti software. The results show progress in community participation and intersectoral coordination, but also structural limitations such as lack of transparency, low decisional autonomy and conditioned budget execution. Institutional effectiveness was closely linked to the technical leadership and managerial capacity of the directors. It is concluded that the National Multisectoral Health Policy represents a strategic opportunity to improve territorial governance in health, provided that more collaborative and decentralized models are promoted, with solid accountability mechanisms.
